About Us
There is a growing movement across the United States to fundamentally change the way care is provided in all senior living environments. "Culture Change" is the term that is used to describe the transformational change that many nursing homes are  embracing. The culture change movement recognizes the need for nursing homes in the continuum of care, but seeks to dramatically alter the way in which that care is delivered in all senior living environments.
Solicit and encourage stakeholder discussion to identify the barriers and challenges to achieving person-centered care in all settings, and actively seek to address these through practical initiatives and projects.
 
Develop, establish and maintain a statewide repository of best practices, fact sheets, educational and training opportunities to promote person-centered care and services for older adults, providers, caregivers, family members and other stakeholders.
Provide an open and inclusive forum to share the values, principles and best practices that support person-centered care and services for older adults, throughout California, regardless of where the older adult resides.
Our Goals
Person-centered care requires that people make a commitment to altering the way we care for older adults. The goals for person-centered care include the following:

All older adults will have access to quality care and services that maximize their independence and self-determination in whatever setting they choose.

To promote person-centered care best practices by providing practical guidelines and tools, expertise, and educational opportunities; and developing and disseminating information to benefit older adults, providers, caregivers, family members and other stakeholders.
