Regional Learning Collaboratives

What is a Learning Collaborative?

The adoption of culture change practices is both exciting and challenging. It is natural for staff to want to fall back on the tried and true and to have concerns about change. The learning collaboratives are designed to provide your team with the coaching and mentoring you need in order to adopt successful strategies of transformational change.

The Regional Learning Collaboratives provide the opportunity for a team from your nursing home to learn theory and then apply it in practice over time. The teams will meet for a year – with quarterly learning sessions and monthly support meetings held between the quarterly learning sessions.

  1. Four Quarterly Learning Sessions will feature a presentation by an expert faculty member. Continuing education credits will be offered.
    • Learning Session #1: The Process of Change
    • Learning Session #2: Person-Centered Care Practices
    • Learning Session #3: Putting the Home in Nursing Homes
    • Learning Session #4: Creating Community

  2. Monthly Support Meetings

    The purpose of the Support Meetings is to follow up on action plans, coach and advise on implementation, and to talk about what is working and what is not working. These three-hour sessions will meet in locations that are central to participating facilities.

The learning collaboratives are designed to provide you with the coaching and mentoring you need in order to adopt strategies of transformational change.

Who should attend?

Plan to bring four to six members of your nursing home team to participate throughout the year. It is best if team members can remain consistent throughout the year; however, it is realistic to expect there will be some turnover of team members.

Choose team members for their leadership skills – not just their position in the organization. It’s also good to bring people who are vocal in their concerns about making change – the ones who say, "That can never work here". We suggest the team brings at least one certified nursing assistant.

What Will It Cost?

The charge will be $999 per facility for the year. The charge covers the costs of the faculty, lunch and printing curriculum materials.

Discount: Register 4 buildings get the 5th free and 25% after 5 homes are registered.

Will Continuing Education Units Be Offered?

Yes.  RNs, LVNs, Nursing Home Administrators and Certified Nursing Assistants can all earn CEUs for attending the sessions.
